NURSE DISMISSAL AND RETURN TO SCHOOL AFTER ILLNESS/INJURY
When your child is not feeling well or is injured during the school day, FPS School Nurses are available in the School Health Office to assess them and provide basic care. The School Nurse’s assessment determines when a student in need of a nursing dismissal from school. Their goal is to offer treatment that enables your child to return comfortably to the educational setting. If dismissal from school is indicated, or follow-up with a physician is recommended, the school nurse will contact the parent/guardian(s) listed on the Emergency Contact Form with their recommendations and arrange for your child’s dismissal from school. We ask parents/guardians to please have a plan in place to pick up your child in case they need to be dismissed from school for illness or accidental injury. In an emergency, 911 will be called, your child will be given first aid and transported to the nearest hospital if needed. You, or a designated emergency contact, will be notified immediately.
Hazel Health has partnered with the Fitchburg Public School District to expand student health services. When your student is feeling sick, they can connect with Hazel for same-day care.

Suicide Prevention Life Line provides an emotional support lifeline to anyone thinking about suicide. For support in English call 1-800-273-8255 and in Spanish call 1-888-628-9454. For additional support lines or to get help via chat visit Suicide Prevention Life Line.
Ok2Talk.org is geared towards providing mental health resources to youth. Includes offering the opportunity for teens and young adults to submit their stories.
The Substance Abuse and Mental Health Administration (SAMHSA) offers resources to get support or find a treatment facility, visit https://www.samhsa.gov/.
The National Alliance for Mental Health offers a helpline M – F, 10 am – 8 pm EST. Call 1-800-950-NAMI (6264) to reach a HelpLine volunteer who can answer questions, provide support and give guidance on practical next steps.
The Asian Mental Health Collective offers international resources and crisis lines including one that provides support in Asian languages: call 1-877-990-8585.
